Before Your Appointment
To avoid long waits, we encourage you to make an appointment for services. If there is a need for same-day services for illness, please call your preferred clinic as early as possible.
When making an appointment, tell us a little about why you need to see the doctor, i.e. illness, injury, routine check-up or complete physical exam, so we can make sure we schedule the time you need. Also, be sure to let us know if you are interested in participating in our Sliding Fee Discount Program. Our staff will work with you to make sure you have the right information available and can set a time with a counselor to determine if you are eligible for the program.
What To Bring
- A Driver’s License or Photo ID
- Your Insurance Card (if applicable)
- Address Verification (piece of mail, tag receipt)
- Proof of income (if applicable)
Please Be On Time
Every patient deserves high quality and timely care. In order for us to offer our patients with quality care, it is very important to keep your appointments and arrive on time.
Please let us know at least 24 hours in advance if you are unable to keep your appointment. If you are 15 minutes late, you may lose your appointment and have to wait for the next available opening.
